Listed below are the requirements for registration at Sickles High School. This information is required of all students. If you will bring the proper documents when you come to school, your registration will be processed on that day. A parent or court appointed legal guardian must accompany all students at registration.

If you are coming from a public school in Florida you must have the following:

1. Report card or copy of transcripts from the last school attended. We will send for your permanent record.
2. Withdrawal Form from previous school if school is in session.
3. Verification of parent’s address. This must be one of the following:
a. Only current TECO electric or water bill. No other bill will be accepted; or
b. Signed Lease or Property Settlement Agreement.
4. Birth certificate or court appointed legal guardianship papers and parent/guardian identification.
5. Immunization records showing proof of proper immunizations, including TD booster, 2nd MMR, and Hepatitis B series (HBV).

If you are coming from ANY private school or a public school outside Florida, you must also have the following:

1. Current (within last 12 months) Florida physical examination (yellow form from doctor).

Additionally, discipline and attendance records will be reviewed if student is transferring from another Hillsborough County school. Depending on the records, it may be necessary to meet with an Assistant Principal prior to registration. 7’
